Getting here: Along the main road Porrettana (SS 64), toward Pistoia, about 1km from Porretta, after the spa. Car park available. Nearby, there is a bus stop for the coach line Porretta - Venturina.

The church is on the main road Porrettana, which leads to Pistoia; it has a very interesting octagonal plan. This Madonna has been worshipped since ancient times and is still very popular amongst locals.
Some say that this Madonna started to be worshipped in 1249. The oldest element of the sanctuary was a wayside shrine with a Madonna painted on the rock. The original building was then transformed into an oratory between 1578 and 1585. Every year, the Madonna is being transported on a procession to the parish of Porretta and then back to the sanctuary of Ascension Day, a tradition similar to that of the Madonna of Saint Luke in Bologna; here, this tradition started in 1613. The church was rebuilt in 1855, but the reconstruction was very hard and time consuming because of the rocks of the mountain. The church houses some interesting paintings by Bolognese artists, like Samoggia and Guadassoni, and several ex-voto inspired by the miracles attributed to the Madonna del Ponte. -
